The following three forms are Advising forms
These forms are to be filled out and presented to the Graduate Advisor along with
a printout of your transcript (obtained from the Graduate Coordinator) when
you are advised. Also, whenever you are discussing your courses or degree
plan with the Graduate Advisor via email or in person, it would be helpful
if you would complete the appropriate degree program form and include it in your email
or bring it with you to his office.
